Please check with your insurance provider to confirm you have massage therapy coverage by a Licensed Massage Therapist in a Massage Therapist's office. Many plans only cover massage therapy if done in a Doctor's office under a Doctor's License. Regardless of coverage, you may still qualify for a discount.

Prior to scheduling an appointment, Hartrum's Massage & Fitness needs to confirm/obtain Pre-Authorization from your Insurance company - This is REQUIRED for any insurance benefits or discounts. • Copy of Insurance Card - Front & Back
• Copy of Massage Benefits Page from Insurance Plan
• Your Date of Birth
